Well Being Check for Seniors and the Disabled

The United City of Yorkville Police Department is currently implementing a new program that will assist seniors and the disabled in daily well being checks. If you are a senior, are disabled, or have a parent who lives alone, this program will help assure you that everything is OK--it's just a phone call away.

Kendall County TRIAD and the Yorkville Police Department is offering this service at no cost to township residents age 55 or older, or disabled residents. If you live in the following townships: Big Grove, Bristol, Fox, Kendall, Lisbon, Little Rock, Na-Au-Say, Oswego or Seward you are eligible to participate.

To participate in the program, please fill out the application form and send it to the Kendall County TRIAD office at 908 Game Farm Road, Yorkville, 60560. For more information you can contact Kendall County TRIAD at 1-630-553-5777.

The program provides the following:

  • Daily telephone calls
  • All calls are automated
  • A warm, friendly voice addresses you by name
  • You choose the days and time you want your call
  • If you don't respond, we let your friends and family know
  • You can call in to stop your calls for the day, or while you're away

How it works

  • You will receive an automated call from TRIAD at the time requested per your application form (example: 10 a.m. Monday-Friday). When you receive your call, you are asked to press 1 if you are well, and 0 if you need help. If you don't answer the phone, or don't press 1, we try again later. You decide how many attempts are made.

Keeps you in touch with family and friends.

  • If you still don't respond that you are well, we contact those individuals that you asked us to call. We proceed down the list until one of your contacts responds that they have receive our call, and will look in on you.
  • Your well-being is important to us and your family. This program will provide peace of mind to your family, friends and you about your well-being.
